Two things which stands for me in Risk management is the "What If" analysis and "Pressure Testing". Typical approach to follow is: 1. Prioritize the actions by ranking them based on the impact. 2. Add risk and mitigates basis the 'what-if' scenario (including human/non-human errors possible) 3. Now do further 'pressure test' on mitigates to assess if anything major is being missed. 4. Finally add some buffer time to manage the risks well.

Risk Management tends to be a tick box a lot of people perform without giving it the quality it deserves! A robust risk management plan not only provides oversight to stakeholders but helps reduce scope creep, keeps schedules on target, reduces stress and leads to an all-round more successful and quality delivered project. Lack of clear risk mitigation leaves workers stressed when situations arise which leads to a cost in either schedule, budget or quality. Appropriate and early intervention is always necessary so that risks have a known budget and schedule implication, less stress and facilitates a quality outcome even if not the originally intended one.

The project charter/plan is always a time-consuming process since it has to be penned carefully and proctored even more carefully. The project charter should be subject to extensive review and validation since it would serve as the foundation/base for the whole project or in other words would serve as the preamble for the whole project. Since the project charter is a document of utmost importance and any minute change should be recorded and documented in it making it a "living document", it should be validated extensively. Companies apply methodologies like Six Sigma in this process which is highly effective in Quality Assurance and sustenance. The 7 QC tools may also be put to effective use to ensure the effectiveness of the charter.

It would be valuable to expand on this point by explaining the importance of effective issue resolution and change management. It is important these are reviewed contemporaneously and with input from SMEs. Lessons learned and round table discussions can be invaluable for providing these kind of updates and identification of new risk. Projects are not a one man operation though so always draft a RACI and ensure you appropriately engage those stakeholders.

Lastly, 1 final thing to consider is detailed regular tracking - make it someone's job to own the tracking and highlighting a red flag if anything goes out of the plan!
