The TMs cannot be blindly relied upon. Every time there is a 100% or fuzzy match, the double check is required. The best practice is to have senior translators checking the translation done using CAT tool right in the CAT tool file, after the translation completion, but prior to exporting the document. It is possible in ideal situation....in real life TMs are often a mess due to lack of time/resources planned by management for TM management (in most cases not considered at all at project planning phase).
