Don't take simulations as the starting point. Build security culture, and then have regular simulations. Only an "aware" and "mindful" staff would engage & perform during simulations. This leaves the employees embarrassed, and more dis-engaged than they initially were. Test your organization, and not your employees during simulation. I will always support monthly phishing campaigns used within a company with feedback depending on how the user responds to the campaign. This may cause some users to see phishing emails and ignore them due to them thinking it is a part of the monthly training and not an actual phishing attempt.
