1 D1: Form a team

One of the root cause analysis process is the 5 whys analysis. We can document the process of asking “why” multiple times to identify the root cause and finally include the final root cause and supporting evidence in documentation.

Establish a Centralized repository/ a shared platform or system to store all documents related to the process. This included reports, diagrams, action plans, and verification results. Also capture insights gained during the problem-solving process. Document what worked well, what didn’t, and recommendations for future improvements.
