As an accident investigator I had encountered a real situation where a person lost his life just for a small error along with very costly equipment. Total pieces of equipment 20, modification for a very critical sub assembly was carried out on 19. It was a safety modification. The operator encountered a real time emergency. The modified equipment just needed one pull. Whereas the unmodified ones required two pulls. Unfortunately the operator was using the only unmodified one but referring the publication of modified equipment. One pull, waiting for response whereas it needed two. Unfortunate very unfortunate, one precious life and millions of USD lost. The situation was reconstructed reliably. Pure Human Error, Not Updating Publications.

The way I tend to approach safety issues is with checklists that go hand in hand with SOP (Standard Operating Procedures). In my own day to day service in the field I find that having a list/itinerary of goals for the day is an almost fool proof way to get it all done. Our company always looks for input as do I at every monthly meeting….and conversations between those. Let’s face it, twelve times a year to hit on safety just doesn’t cut it. Be safe out there in all you do. That’s my Mantra. When in doubt, reach out!
