What resources or supports can they access? Are there any gaps or barriers preventing them from using these resources? What strengths or assets can help them overcome these problems? And what goals or expectations do they have - how realistic or achievable are they?

5 Collaborate with the client

The final step of the assessment is to collaborate with the client and develop a plan of action. You should share your findings and feedback with the client, and invite them to comment, question, or correct them. You should also discuss the possible options, strategies, or interventions that can help the client achieve their goals or address their needs. You should respect the client's autonomy and self-determination, and involve them in the decision-making process. You should also agree on the roles and responsibilities of each party, and the time frame and criteria for evaluating the progress and outcomes.

6 Review and update the assessment

The social assessment is not a one-time event, but an ongoing process that requires regular review and update.
