The importance of actually understanding why cannot be understated. I have seen too many times where an assumption of the line went down because X was proven wrong because of the lack of specificity within the initial problem statement. In many cases bringing together a group of experts at the beginning of this process will allow for a better problem statement and a faster resolution.

In many cases the time from an issue occurring to a solution hypothesized and tested can be days. Remember it's more important to be correct than to be quick.

These 5 steps are normal RCA and doesn’t touch base on how to do it with urgency, I was intrigued and wanted to learn how but found it not as I expected. In my opinion, corrections we can do with utmost urgency such as rework, replacement and waivers but RCA will need some days to compile and really be sure it IS the root cause you want to answer to the customer’s CAR.
