The use of knowledge management tools such as pre-mortems and post-mortems can support learning and innovation. Before implementing an initiative tries to understand what are potential causes of failure and/or conditions that could derail the project or initiative. This enables knowledge and learnings from other initiatives to be brought to bear in the preparation process and make relevant adaptations. The post-mortem occurs after the project or significant steps or stages and seeks to understand both what went well what didn't go as planned wrong as well as unexpected outcomes. It is important to understand the causal factors behind the outcomes including success. These 2 approaches are scalable & adaptable from small to complex projects.
