It is a practice in our industry that when a new process or equipment is being introduce or when a certain process working environment or work procedure is being altered to increase productivity, Our team of Safety practitioners is being requested to assess the risk of possible hazard that may contribute to accidents and implementing the most practical Engineering controls to isolate the identified hazard to avoid the workers contact or exposure to that physical or health hazards.

An effective incident analysis start with an effective accident/incident investigation. By knowing what really transpired will be your starting point for your root cause analysis which will give you the idea which factor contributes the accident/incident. Thus, knowing the contributing factor will now give you effective way to address and control the hazards.
