In my humble opinion, ERPs never fail, its the team failure in which lack of adoption & refusal to accept change is the biggest factor as ERP implementation is always a cultural change. Also everyone has different parameters to judge ERP success or failures.

I know some companies wich have implemented ERP and were not successfuly because they did not define the process before. It is also vital for the implementation that the companies MUST consider some consultancy hours in the BUDGET, because operational issues always arise after the implemantation.

The solution can anchor around ERP, but the core components are Process, Data, technology and people. ERP is not transformative, it helps transformation. When the above two lenses are applied incorrectly, ERP will fail. In my experience, if an ERP implementation project includes a hypercare as part of the plan, there is a fundamental assumption that something will fail. Why would anyone move into a skyscraper when once has to be prepared for severe contingencies? Makes no sense.
