Here's how you can effectively handle and resolve failure in optical engineering.

Verify the correct components have been used. The correct fiber, connectors, lazers, splitters, etc...and clean clean clean. Then sectionalize. Use calibrated test equipment to insure accurate readings as you OTDR and light meter test. As you test and document each segment, you will be able to line it up with the design. Are the numbers in spec? Is it the fiber or the equipment that is not meeting spec? Once you determined the problem(s) fix and retest one segment at a time.

By failure, I'm presuming that the system is already been in service for a period of time. OTDR test for an optical fiber link failure should be performed. Optical handoffs, fiber jumpers and connectors should also be examined and replaced if not meeting recommended tolerances. An overall end to end test would then need to be performed to ensure that the system is now up and working.

I find it difficult to answer this question because it's not one I have had to make. I have not had a "failure" in optical engineering. In fact the most common issues regards the Opto-Mechanical engineer/designer, ie implementation. This gives the reason for my policy of designing/capturing critical 3D geometry (as CAD exports of components and ray trace exports) and mounting strategies designed in SolidWorks. Wavefront error measurements are not revealing when it comes to identifying the issue. Wavefront gives a measurement of the system without much data to correct it. Review of the design should occur prior to any implementation. If you don't know your design down to the finest detail, then you're inviting "failure" to the party..

In my experience we have a tendency to over rely on our setups, i.e. once we have a test up and running tested under a limited set of conditions we assume anything wrong now lies in the DUT. But in many cases an error led to a wild goose chase the ended with realizing the setup was at fault, things like: Setup stability - optical setups can be notoriosly unstable over time - mechanical fixtures get loose, people bump on the optical table, cables get bent, temperature fluctuates. Test Device doing unintended actions - VNAs might change internal resistivity for higher frequencies, some devices will strive for stability when you wanted a deviation.
