Start with a conversation. There’s always a reason for a decline in productivity. They may have personal issues that require their attention. Lack of skills and knowledge may be a reason too. Then, act accordingly. If the reason is personal, ask how much time they need to resolve it and see what you can do to help. Adjust the workload or suggest them to take time off. If they lack knowledge, suggest additional training. Some cases may require you to build a performance improvement plan with clear goals and regular checkpoints Remember to stay human. It’s likely that they understand they’re not at their 100% and already feel guilty, don’t push too hard. Explain that you want to help them while sticking to your team commitments.

For the individual. For the individual who's missing deadlines, the way I have tackled this is by 1. Opening a dialogue. 2. Being transparent. 3. Setting expectations. 4. Providing a platform to improve. 5. Coaching, Training and Mentoring to help improve.
